Co-Hosted by the Aboriginal Chronic Conditions Network and The Aboriginal Health & Medical Research Council, this year’s conference aims to showcase each other’s success and key learnings in the design and delivery of chronic care models for Aboriginal peoples and communities.

Speakers will include representation across the NSW Health Local Health Districts/Specialty Health Networks, Aboriginal Community Controlled Health Services, Primary Health Networks, University Partners and other Non-Government Organisations.
